4: ASSESSING THE PERMIT-TO-WORK SYSTEM ARRANGEMENTS
4) Based on the scenario only, comment on the food manufacturing
site’s application of the permit-to-work system. (Marks 10)
Answer: Looking at the scenario it seems to be positive
and negative comments on the food manufacturing site’s PTW system.
Let’s first have a look on the positive comments on site PTW
system according to the scenario:
It is observed that the work carried out by the contractor on
that day was under the authority of PTW system. The contractor is trained in
the responsibilities and well aware of the acceptance of PTW and how it
operates, when the worker presented a job description note to the site PTW
issuer a brief discussion occurred between both of them. The site PTW issuer
discussed with the contractor in detail regarding the nature of job and
clarified the understanding of the hazards and risks involved in the proposed
activity. The self-duplicating PTW forms were duly completed and authorized
with relevant signatures. There were 3 copies of PTW issued for each task of
which 1 copy was displayed on the job site and 1 was handover to the worker and
1 is kept at site office for records. PTW was issued for 8 hrs. time frame that
day from 9.00-17.00. The site permit issuer also informed the worker about the
associated control measures, isolation of equipment’s and to wear appropriate
PPE before staring the work. The management has assessed the competency of the
contractor and they have vast experience in contracting work including the PTW
system. A job specific induction was provided to the contractor from the site
supervisor before starting a job. Considering all above it means that a
positive PTW system prevails on site working condition under the influence of
HSMS.
The negative comments which has been noticed at the food
manufacturing site as per the scenario are:
The site PTW issuer were extremely busy as they were issuing 75
PTW’s a day. Due to heavy work load PTW issuer was somewhere somehow neglecting
his other responsibilities which were essential at the food manufacturing site.
According to the PTW issuer the task appeared to be of low risk and due to busy
schedule he did not physically inspect the area where the task was due to
perform. A single mistake and little negligence has made the contractor
suffered a knee injury as this will impact negatively on companies HSMS.
