3: DETERMINING ROOT CAUSES OF THE ACCIDENT
3) You try to inform the
supervisor that the root causes of unsafe behavior are usually management
failures.
Based on the scenario only, what management failures are likely to
have contributed to this accident?

Answer: The Management failures that has been contributed to the
accidents according the scenario are listed below:
The senior management
delegates Supervisor and long serving Middle Managers carries casual attitude
towards BSA program and they are not convinced from it therefore BSA program
will need careful management for Supervisors and other managers to accept it.
This a HSE negative indicator shown by the top management representatives.
According to the
supervisor investigating an accident is a waste of time and any reasonable
person would come to the same conclusion because he thinks incidents always
occur die to the human error. He also thinks that most of the incidents and
accidents are unavoidable, and this is because of the workers unsafe behavior
and they should be blamed for it. Blaming individuals will also demoralize the
workforce and made the workers feel insecure from the risks of health as the
injured worker said “we live with the risks and accept the consequences without
complaint”. This is because of the management negative attitude towards HSE.
There was no adequate training,
TBT etc. provided to the worker for the task as he said only induction training
was provided and site supervisor was not around later that day. While carrying
out incident investigation it was found that the oil is leaked on the board and
platform at incident spot. The incident spot was not physically examined and task
was not carried under supervision.
Permit issuer thought
the task is low risk and was also very busy as he issued 75 permits that day
which lead them not do a physical check where the task was performed.
Employer failed to
provide adequate safety equipment’s as the work slipped due to poor grip of
safety shoes. This incidents also indicates that oil spill skits were not
provided and poor housekeeping was also observed.
The investigation concluded that there were
many organizational management failure and identified only one human error.
